 Residential Daily Services Tracker

Residential Daily Services provide individualized support and supervision to help individuals with intellectual and developmental disabilities live safely, successfully, and as independently as possible in their homes and communities. Services are delivered according to each individual's Person-Centered Support Plan and are designed to meet their unique needs, preferences, strengths, and goals while ensuring their health, safety, welfare, and rights are protected.

 

These services include assistance with activities of daily living, personal care, health and medication support, meal preparation, transportation, community participation, relationship building, behavioral supports, and household management. Caregivers also provide teaching, coaching, and guidance to help individuals develop, maintain, and improve skills that promote greater independence, self-determination, and quality of life.

 

As part of Colorado's Developmental Disabilities (DD) Waiver program, Residential Daily Services serve as a community-based alternative to institutional care. The focus of the program is not only to provide supervision and support, but also to help individuals actively participate in their communities, pursue personal goals, strengthen natural supports, and achieve meaningful outcomes while living in the least restrictive environment possible.

Thorough documentation is key! Check out some examples of strong documentation here. Example Documentation Strong: Goal: Maria will prepare a simple meal with verbal prompts 3 out of 5 opportunities. Support provided: Caregiver provided step-by-step verbal prompts for making a sandwich. Maria retrieved bread and turkey independently, then needed 2 verbal cues to spread mustard. Completed the task successfully. Member response: Maria was engaged throughout, smiled when finished, and said she did it. Progress: Goal met today (4th success this week). Progressing toward 3/5 target. Adequate: Worked on meal prep goal. Maria made a sandwich with help. Did okay. Weak: Worked on goals.

Did the client sleep in their home?
Did the client receive Respite care today?
Did the client participate in community, social, educational, or employment activities today? Example: Attending school, work, a day program, or other scheduled activities.
