SUPPORT SERVICES

Families For Families is pleased to offer the following support services at this time: Individual Residential Services and Supports (24/7 in-home support), Supported Community Connections (one-on-one community-based day program), Non-Medical Transportation (typically used for transportation to and from day program), Respite (primary caregiver relief) and Mentorship (self-advocacy training). We find that these services are heavily utilized by DD (comprehensive) and SLS waiver recipients and are delighted to provide these services to the community.  Our agency seeks to expand choice among the recipients of these services by expanding their provider options and are honored to provide these supports. It is our goal to have a meaningful impact and enrich the quality of life of our clients by providing these much-needed services. Through all these support services, our primary overarching goal is to encourage and execute the personal choices of those we serve.

Individual Residential Services

INDIVIDUAL RESIDENTIAL SERVICES & SUPPORTS (IRSS)

Residential services and supports are provided by our agency to assist participants of the program to reside as independently as possible in the community:

Community Connections

COMMUNITY CONNECTIONS

Our agency provides support to clients that allow them to participate in community activities. Specifically, Families For Families facilitates consumers participation in community activities by integrating their Service Plans and individualized goals as much as possible while outside of the home.

Non-medical transportation

NON-MEDICAL TRANSPORTATION

Our agency provides Non-Medical Transportation services to consumers in order for them to gain access to waiver and community services, activities, and resources according to their Service Plan.

Respite

RESPITE

Our agency provides respite services on a short-term basis to relieve the primary caregiver for our clients. At this time, we only provide respite services in the respite provider’s home.

Mentorship

MENTORSHIP

Our agency provides mentorship services to promote client self-advocacy; this is achieved by instructing, modeling, advising and teaching through experiences. Some activities that mentorship services aim to affect are selecting care providers, understanding issues surrounding their health, and community organization participation. These services are performed wherever the consumer needs them, in the home or when out in the community.
